Clear terms.

These terms describe the FORM / CASE creator program operated by FORM CASE LLC. The program is for creators aged 18 or older. Participation requires accurate information and acceptance of these terms.

Your follower code

Your creator code gives followers 10% off eligible merchandise. Your creator commission is 15% of eligible merchandise revenue after discounts and refunds. Tax and shipping are excluded from the commission calculation. Codes cannot be combined with other offers. Your own orders do not earn commission.

When commissions are paid

Commissions have a 30-day hold before becoming eligible for payment. Eligible balances are paid manually through PayPal on the 15th of each month, with a $20 minimum. Balances below $20 carry forward to a later payment cycle. Refunds reduce the eligible commission balance; refunded merchandise does not earn commission. Saving a PayPal email does not itself send a payment.

Your partner case

A partner code gives you 25% off one case, one time. It cannot be combined with another offer. Partner-code orders do not earn commission, and self-orders are excluded from creator commissions.

Honest recommendations

Clearly disclose your relationship with FORM / CASE when sharing a creator code or recommending a product. Keep descriptions and claims accurate, follow the rules of the platform you use, and do not send spam or misleading messages. Do not imply that you represent FORM CASE LLC or make promises on its behalf.

Participation and changes

You may stop participating at any time. FORM / CASE may end participation or disable codes, including for misuse, inaccurate information or failure to follow these terms. Ending participation does not change the calculation and payment rules above for valid, eligible commissions already earned.

Your private details

Your PayPal email is encrypted and stored in the private backend for payout administration. Amp receives only whether the payout-details step is complete, never the email itself. Keep enrollment and payout links private. Contact us if a link is lost or you need to update your details. Our privacy notice describes other store information practices.
