These terms apply to purchases from FORM CASE LLC, operating as FORM / CASE, at shopformcase.com. We ship to supported addresses within the United States.

Your order

Check your exact phone model, case style, quantity and delivery address before paying. Prices are in US dollars; the checkout total shows shipping and any applicable tax. After payment is confirmed, your order is automatically sent for production. We email tracking when it ships. If we cannot fulfill it, we will contact you to arrange a suitable resolution or refund.

Made-to-order products

Product images illustrate the selected case. Screen colors and physical print placement may differ slightly. A phone case cannot guarantee protection against every impact, scratch or accident. Choose the magnetic case style if you require its built-in magnetic module; the standard Tough case does not include magnets.

Delivery, cancellation and returns

Our shipping and returns policy explains estimated timing, cancellation requests, model selection and how to report a problem. Mandatory consumer rights continue to apply.

Welcome offer

When available, our verified email signup offer gives eligible new customers 10% off their first order, one use per customer. Standard US delivery is included; tax is extra. The offer cannot be combined with other offers. Verify your email and review the discount in secure checkout before paying. Previous customers and customers who have already redeemed the offer are not eligible. Removing an offer from your browser does not unsubscribe you from marketing emails.
